Fairview is working to re-envision how we provide the best care for patients during a time of transformational change in health care. Fairview clinics have launched the care model innovation project—a ground-breaking, patient-centered, team-based approach to delivering primary care. Its goals are threefold: to improve clinical outcomes and the patient experience while reducing the cost of care. The result: better value for our patients, partners and the community.

Among the hallmarks of care model innovation is a shared sense of leadership among nearly everyone who interacts with our patients. We’re doing our work differently—empowering staff in a spectrum of roles to make decisions and create unique solutions that work for their patients in their environment.

Our providers appreciate the closer relationship with colleagues and improved communication and culture that care model innovation offers. Clinicians and staff have more proactive, ongoing, individualized and engaging interactions with patients about their abilities to stay healthy. Ultimately, they are able to spend more time with patients because each patient’s needs can be addressed by a number of clinicians on the team.

This approach to delivering patient care has asserted Fairview’s position as a leader—to our patients, community and government.
